An explanation of the quarterly reporting requirements for non-profit and other agencies funded through Community Services Department.

Quarterly Financial Report

Quarterly each agency is responsible to provide a line item report that includes all program revenues and expenditures for the quarter. 

  • This report is due for each funded program on or before the 30th of the following months (for the quarter ending the prior month):  Jan, April, July, and October. 
  • This report is accessible through the online data management system (ODM). 
  • Be sure to report all revenues and expenditures accurately for each program.
  • NOTE:  The agency will need to create a new report ONCE each fiscal year and add to that report each quarter.  Please remember to change the date submitted and the timeframe of the quarter reported.

Quarterly Program Outcome Measurement Report Form

Quarterly each agency is responsible for tracking and reporting outcome results for each of their programs on the Online Data Management System (ODM). 

  • These reports are due on or before the 30th of the following months (for the quarter ending the prior month): January, April, July, and October. 
  • Feedback will be provided for your review and to take the appropriate action.
  • Agency will need to "Create New Program Outcome Report Form" each quarter.

Quarterly Non-Profit Agency Funding Report

This form is utilized to report any alternative funding sources for which the agency has applied to either offset or replace the funding received from the county for each program. 

  • This report is due for each program on or before the 30th of the following months (for the quarter ending the prior month):  January, April, July, and October. 
  • Report what efforts agency is making to obtain alternative funding for the program as well as the results of obtaining such funding. 
  • Report may include special events, donations or bequests if they are specifically dedicated to the program. 
  • These are reported through the online data management system (ODM)
  • For agencies seeking alternative funding, looking for good capacity building seminars, and other non-profit agency information, one good resource is the Non-Profit Resource Center.
  • NOTE:  Agency will need to "create new funding report" for each quarter.

 

Quarterly Client Demographics Report (Children's Services programs only)

Quarterly agencies must provide a program report that includes the following demographic information on clients receiving services through the program:  gender, race, age range, and residential zip code. 

  • This report is due on or before the 30th of the following months (for the quarter ending the prior month):  Jan, April, July, and October.  These are reported through the online data management system (ODM). 
  • IMPORTANT:  The report for the first quarter of the fiscal year (October - December) should include ALL participants in the program.  Thereafter, in the subsequent reports, only new participants should be reported.
  • NOTE:  Agency will need to "Create New Client Demographics Report" on the ODM for each quarter
