Authorization for the County Administrator, or designee, to execute Change Order #3 to Work Assignment #26 with Carollo Engineers, Sarasota, FL for Lake Manatee Dam Supplemental Investigation and Emergency Priority for an increase of $664,168.00 and an adjusted total not to exceed of $1,658,862.00.

Background Discussion

October 8, 2013, Work Assignment #26 with Carollo Engineers was executed for Lake Manatee Supplemental Investigation in the amount $239,498.00.

On February 14, 2014, a briefing was held to relay preliminary information to the County of results of a supplemental dam investigation by Carollo Engineers and their subconsultant AMEC Environment and Infrastructure, which indicated serious and potentially hazardous conditions at the Lake Manatee Dam.

On February 19, 2014, Change Order #1 to Work Assignment #26 was executed and provided for additional field testing at the dam with no increase to the total cost of the work assignment.

Agreement 14-1467MW, Lake Manatee Dam Emergency Rehabilitation with Hayward Baker, Inc. of Tampa, FL at a total contract price of $14,091,190.00 approved pursuant to emergency procurement procedures and dated April 1, 2014, was ratified by the Board on April 3, 2014.

Change Order #2 to Work Assignment #26 was executed on April 1, 2014, increasing the scope of work associated with the ongoing site investigations and engineering evaluations and to add engineering support during the Lake Manatee Dam Emergency Rehabilitation  construction phases.  The total increase to the work assignment for this effort was $755,196, bringing the total expenditure under this work assignment to a not to exceed amount of $994,694.

Change Order #3 to Work Assignment #26 includes engineering costs due to the discovery of a sheetpile wall north of the service spillway along with TRD alignment and a void system under the spillway approach.  The Change Order also covers the installation of a series of nested piezometers downstream of the cutoff wall for measuring the effectiveness of the cutoff wall.  The cost of Change Order #3 is $664,168 bringing the work assignment total to a not to exceed amount of $1,658,862.00.
