Action Requested
Authorization for the Chairman to execute the Case Management Referral Agreement for the Assisted Living Medicaid Waiver grant program between Manatee County and West Central Florida Area Agency on Aging, Inc.

Background Discussion
  • Case Management Referral Agreement allows for the Community Services Department to continue providing Case Management services to clients enrolled in the Assisted Living Medicaid Waiver (ALW) grant funded program.
  • Manatee County has been the Case Management provider for the ALW program for over 20 years.
  • The Referral Agreement allows for the county to continue providing case management services without disruption.
  • Case Management services are paid for through dedicated federal and state funds assigned to the ALW program.
  • There are currently 43 active clients being case managed under this program.
