Action Requested

Motion to approve the following recommendations for the Manatee County Indigent Health Care Services Program:

  1. Reimbursement of Advanced Practice Registered Nurses (APRN) and Physician Assistants (PA) in non hospital-based settings at the respective Medicare reimbursement rate provided a Florida-licensed physician (M.D. or D.O) is on-site during the episode of care.
  2. Expansion of radiology and laboratory reimbursement to include non hospital-based agencies.
  3. An alternating weekly reimbursement schedule of hospital-employed and/or hospital-contracted providers' service claims then non hospital-affiliated service claims, in the order claims are received.

Background Discussion

The Board of County Commissioners (BCC) received updates from the Neighborhood Services Department, Health Care Services Section, regarding indigent health care services funded by the Board of County Commissioners. Recommendations to continue ongoing activities that improve access to care for eligible residents of Manatee County by maintaining reductions in average care costs were presented at the BCC's Regular Meeting on August 25, 2020. The Board Chair approved a request to consider the proposed recommendations at a subsequent Regular Meeting of the BCC on this date.

Due to annual reductions in State-funded safety-net hospital services, indigent health funds invested in local community-based safety-net health services are indicated, to avoid the unnecessary use of the emergency department by the uninsured.

Currently, indigent health services provided by ARNPs or PAs are not payable in any setting by the County. The above recommendations increase access to affordable, community-based safety net health services to continue to reduce the burden on the emergency-department and safety-net hospital providers and observed increases in uncompensated care costs.
