Action Requested
Authorization for Chairman to execute Amendment #1 to the 12/13 Alzheimer's Disease Initiative (ADI) grant agreement ADI-12/13-MAN between Manatee County and the West Central Florida Area Agency on Aging, Inc. (WCFAAA) changing language throughout the agreement.

Background Discussion
  • Language in this agreement is being updated to match required/changed language in the agreement between the Department of Elder Affairs and the Area Agency on Aging, Inc.
  • Amendment #1 does not affect client services.
  • Manatee County has served as Lead Agency for the Alzheimer's Disease Initiative (ADI) grant program for the past twenty-two years.
  • ADI grant funds are provided by the state through the West Central Florida Area Agency on Aging, Inc.
  • ADI is designated to provide Case Management and In-Home Respite service to prevent or delay premature institutional placement of Manatee County residents age 60 or older afflicted with Alzheimer's Disease or some other form of severe dementia.
  • Amendment #1 changes some language in the original agreement.
  • Although service provision under the FY 12/13 agreement ended June 30th, 2013, the agreement itself does not end until 9/30/14 for any additional administrative and/or funding changes that need to be made.
