Authorize the County Administrator or his designee to execute Change Order No. 4 to Work Assignment No. 24 pursuant to Agreement No. 07-0759TC with Cardno, Inc., in the amount of $106,181.00 for a revised Work Assignment total of $1,389,469.00.

Background Discussion

Change Order No. 4 to Work Assignment No. 24 includes providing intersection design for the 45th Street and 44th Avenue intersection, pond re-design and permitting for inclusion of the Parcel 128, CCTV camera and pole design, traffic signal modifications at that intersection, establishing the average of all high water heights over a specified period (mean high water) as well as conduct a survey and public information and outreach services.

Procurement History

On June 21, 2013, the Board of County Commissioners (BCC) approved Work Assignment No. 24 to provide design, permitting, survey/mapping and construction services for the widening from two lanes to four lanes of 45th Street East from SR 70 to 44th Avenue East for a total cost of $989,892.00.

On August 13, 2014, Change Order No. 1 to Work Assignment No. 24 was issued to provide one additional traffic signal design and permitting, street lighting design and permitting along 45th Street East, and an additional seventeen (17) legal descriptions and sketches. Change Order No. 1, in the amount of $107,753.00, increased the Work Assignment sum to $1,097,645.00.

On October 21, 2014, the BCC approved Change Order No. 2 to Work Assignment No. 24 to add ten (10) additional Legal Descriptions and Sketches, design and permitting for additional utility relocates, and design and permitting for replacement of a portion of existing gravity sewer. Change Order No. 2, in the amount of $36,087.00, increased the Work Assignment sum to $1,133,732.00.

On January 13, 2015, the BCC approved Change Order No. 3 to Work Assignment No. 24 to add land acquisition and relocation services to 45th Street East from SR 70 to 44th Avenue. Change Order No. 3, in the amount of $149,556.00, increased the Work Assignment sum to $1,283,288.00.

Change Order No. 4 to Work Assignment No. 24 includes providing intersection design, pond re-design and permitting modification, CCTV camera and pole design, traffic signal modifications as well as outreach services in the amount of $106,181.00 for a revised Work Assignment total of $1,389,469.00.
