Physicians, ARNPs, PAs, clinics, community and specialty hospitals, behavioral health centers, surgical centers, hospice, governmental entities, and many others.
*Unfortunately, if a provider is already in a Volunteer Health Care Provider Program (VHCPP), they cannot participate.
How do I submit claims for payment?
All invoices will be submitted on a CMS-1500 or similar form.
What is the reimbursement rate?
Providers will be reimbursed at the current Medicare rates.
What documents do I need to submit to become a Health Care Plan Provider?
Not only will you be providing much needed services to the community, you will also have access to the HIE, making eligibility verification more streamlined.
