Vaccination Appointment Information

The information on this page is for vaccine recipients who have been contacted by Manatee County and booked an appointment for their first dose of the COVID-19 vaccine. Please do not arrive more than 15 minutes before your scheduled appointment time.

What to bring

You will need to bring the following to your appointment:

• Valid form of identification (driver's license, passport, etc.) showing age 65+ and matching your booking information
• Completed Department of Health COVID-19 Vaccine Screening and Consent Form - This will also be available on-site at your appointment.
• Download consent form: English / Español / Kreyòl Ayisye
• Registration confirmation number (provided by 311 during your booking call) - If you don't have this number, your name will be matched with the booking list.
• Proof of Florida residency - You must show a valid Florida driver's license or ID card, or any two of the following:
  • A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement.
  • One proof of residential address from the seasonal resident’s parent, step-parent, legal guardian or other person with whom the seasonal resident resides and a statement from the person with whom the seasonal resident resides stating that the seasonal resident does reside with him or her.
  • A utility hookup or work order dated within 60 days before registration in the medical use registry.
  • A utility bill, not more than 2 months old.
  • Mail from a financial institution, including checking, savings, or investment account statements, not more than 2 months old.
  • Mail from a federal, state, county, or municipal government agency, not more than 2 months old.

What to expect

Arrive no more than 15 minutes before your scheduled appointment time.

Expect to spend about an hour at your appointment. You will remain in your car the entire time.

You will be asked to remain 15 minutes after receiving the vaccine to monitor for symptoms. People with a history of significant anaphylactic reactions to vaccines will have a 30 minute symptom monitoring period.

Frequently Asked Questions

What if something happens and I can't make it to my appointment?

Call 311 to cancel your appointment and have your name put back in the vaccination standby pool (if needed).

Can I carpool / can I drive my parents if I’m not receiving the vaccine?

Yes, you can drive your parents, neighbor or friends. Only those with confirmed appointments will be provided with a vaccine dose. There are multiple internal checks to ensure that we are adhering to our planned allocation each day.

What if I have a reaction to the vaccine?

Highly trained and skilled paramedics from Manatee County EMS are on-site to assist with any recipient(s) experiencing an adverse reaction.
