Action Requested
Authorize the County Administrator or his designee to execute Change Order 5 to Work Assignment #1 with Cardno, Inc., in the amount of $57,258.00 which increases the total Work Assignment amount to $1,255,685.00.

Background Discussion

Change Order #5 to Work Assignment #1 will redesign for turn lanes, bike lanes, and post design services, increasing the total Work Assignment per this Change Order 5 in the amount of $57,258.00, which increases the total Work Assignment amount to $1,255,685.00.

Procurement History

On January 27, 2009, Work Assignment #1 was executed for 44th Avenue East Roadway Improvement from 19th Street Court East to 30th Street East (County Project Number 6045660) in the amount of $887,832.00.      

On February 19, 2010, the County approved Change Order #1 to Work Assignment #1, Design of Roundabout, at a cost of $89,999.00.

On October 23, 2012, the County approved Change Order #2 to Work Assignment #1, Environmental and Florida Department of Transportation (FDOT) Permit and public meeting at a cost of $46,408.00. 

On July 10, 2013, the County approved Change Order #3 to Work Assignment #1, Additional Planning, Engineering, and Graphics, at a cost of $37,540.00. 

On January 13, 2015, the County approved Change Order #4 to Work Assignment #1, Addition of Public Involvement and Redesign of 44th Avenue and 19th Street Court at a cost of $136,648.00.
